a big big suggestion for cognitive decline is Namenda, they use it for cognitive difficultes and usally it does well with a stimulant

if your on adderall, you could have the dose increased, and have it taken 3 times a day to sustain the psychostimulant effect, personally adderall is waker upper, dexedrine is more smooth and less speedy, its more for focus, adderall is more like getting you going in the morning, and staying sharp, it depends on the person's brain anatomy which one to use
