Being that you have pure o could the ruminating about the dose of zoloft be an OCD thought/feeling that you only ebbs when you are having a tough time? I don't think you have anything to worry about as my doctor has said time and time again to me that it will be ok to be on zoloft for the rest of my life. I think it might be partly your pure o to blame for this worry about zoloft.
