Most people do not respond to nortriptyline until they reach a dosage of 75 mg. If you have a fast metabolism, you might need as much as 150 mg. For nortriptyline, it is worth getting a blood test for drug levels. I need 150 mg in order to respond. I am an "extensive metabolizer". My blood levels at 150 mg are still below the maximum. The reference range for the blood test is between 50 - 150 ng/ml.

Nortriptyline is a unique drug among the tricyclics. Unlike the others, one can actually lose the antidepressant response if the dosage is too high. This phenomenon is known as a "therapeutic window".

I think it is too early to be worried at this point. It is unlikely that you would respond to 50 mg, even if you stayed at that dosage for a month.

I hope you are able to recapture you original response to nortriptyline. You'll let us know?
