I'm not sure what kind of explanation you are looking for.

The terse answer to your question is "because it works".

It is just another drug that you may never have tried before.

Why would you expect a guarantee from anyone? I'm hoping that no one here is silly enough to offer one. Were you offered a guarantee by your doctor for each of the drugs he has tried on you?

I think if you were to poll psychiatrists who were practicing before Prozac came along and who were still comfortable prescribing MAOIs, you would find the pervasive opinion that these drugs will provide a greater rate of success than any other class of antidepressant. The reason that they are not a doctor's first choice is because of the dangers involved with maintaining a low-tyramine diet and avoiding certain drug combinations. Unfortunately, some doctors leave it as their last choice and have their patients taking a handful of ineffective drugs instead.

In defense of those doctors who are reluctant to use MAOIs, they have to be sure that a patient is a candidate for taking them. It takes a certain amount of functionality and responsibility on the part of the patient to manage their diet and drug restrictions.
