I would say the hell with theory and just know that it works for some people. For me, it was Effexor that I combined with nortriptyline. It was only partially effective for me, but the response was stable over time. Right now, I have combined nortriptyline with Nardil.

Yes. You are in agreement with Steven Stahl, MD.

Again, I would work the other way around. Take it now and worry about theory later.

It works.

I imagine it would work with any of the SRIs at similar rates. Perhaps Lexapro would be best as it is more selective than the others. Uh, oh. Theory again.

I am not sure. Possibly. In the same vein, Effexor and probably Pristiq also have DA reuptake inhibition properties.
