The extreme dizziness/fainting has gotten much better in the past couple of weeks and the daytime fatigue has improved marginally (though this is by far still my #1 complaint about Nardil).

Should I push my luck and try to up it to 90mg and get up to feeling as good as possible (though I worry about more side effects); should I stay where I am and wait for the side-effects to subside and just be happy with some pretty solid antidepressant effects or should I consider reducing back down to 60mg to hopefully get rid of the side-effects and leave myself more room to up the dose in the future?

I was in your boat, and I JUST increased my dose to 90mg. I was also feeling pretty good at 75mg. I never had real problems with dizziness, and never passed out. It also doesn't make me tired, if anything I have more energy now. I have no serious side effects at all actually. I decided to go to 90, because the research says 1mg/kg is the ideal dose for most patients IF they are tolerating the med OK. It sounds as if this might not be the case with you. I wouldn't have increased my dose if I had the same side effects you are having. Maybe wait a month or two longer and see if the side effects resolve at 75mg, and then make a decision? The one benefit of staying at 75mg is that if you start to feel depressed again, you can increase your dose. It's not an option once you are at 90mg.
