Of course, follow your doctor's directions. But my general advice would be that once you're up to 45mg, stay there for at least at four full weeks to see if you're getting any response. Six if you can stand it. If after this time you're getting some response, you'll need to determine whether you're continuing to improve (even slowly), or the response has stalled. If you feel you've stalled, I'd say increase by 15mg and wait another four full weeks. Lather, rinse, repeat.
Don't obsess about raising your dosage just because it seems low compared to some of the other people's dose here. Just assume that 45mg will work, and if it doesn't, you can always raise it later. Don't go too high too fast with the assumption that once it starts working, you can lower it later. This risks a relapse of depression/anxiety, and you really don't want that.
I hate waiting for these things to kick-in as much as the next person, but in the long run you're going to be better off with the bare minimum therapeutic dose.
Now a word of warning: You probably already know that Nardil will increase your appetite. What you need to know is that you may not realize the extent of it until you've already gained a bunch of weight. Trust me on this one, it's really insidious. As soon as you start the Nardil, you need to be vigilant about not only what you eat, but how big your portions are. If you normally eat one donut, on Nardil, you'll find yourself eating three or four, or six. And you won't even realize it at the time! So be extra careful.
