My guess would be that she was reacting to a decrease in anxiety, which some ADs can produce more quickly, and which can be affected by the placebo effect as well.

If her problems really did go entirely away on the first day, and it's not placebo effect, then I would consider it highly likely that her problem wasn't Major Depressive Disorder in any flavor. Maybe something more Axis II? Or even whichever Axis general life issues are listed under...
