Have you tried Anafranil (clomipramine), a TCA that largely acts as an SRI/SSRI? It used to be the "gold standard" for OCD until newer SSRIs have replaced it. It still stands out for some treatment-resistant OCD.

When I switched from Luvox to Cymbalta, my pure obsessional thoughts (garbage words) which are annoying but on the swatting at flies level increased. But that's just my experience -- everyone is different. I think it was just that Luvox is an excellent medication for OCD at a high dose. Cymbalta provides fairly good response for me for depression, but not as much for OCD.
