A lot of people with OCD are tormented by thoughts that they will hurt those they love--physically. It has no connection with reality. Someone with OCD, who is tormented by those thoughts, will certainly never carry through with them. Do you have any accompanying rituals when you have the thoughts?
Often SSRIS in high doses are prescribed for OCD. It sounds like you are on a low dose of Zoloft.
