We're in the process of increasing my daughter's Zoloft dose from 75mg to 100mg for Social Phobia. We switched from Lexapro which had a partial effect on SP because of extreme fatigue and memory issues. She tolerates Zoloft much better.
We didn't notice much of an improvement in her SP symptoms past 50mg of Zoloft. Her interaction improves about 30-40%, but she's stuck with long standing negative self perception and thoughts. She's been doing CBT for over two years, but it doesn't seem to generalize.
Is it reasonable to increase the Zoloft dose more even though we've seen this levelling off of effect? We saw the same sort of plateau in effect with Lexapro, though raising that dose made her a zombie.
At what point do you say that SSRIs are just going to be partially effective and decide to augment with something else? I'm afraid that the suggestion of trying Prozac will come up and I don't really want to waste another several months or a year trying that out if there's no reasonable expectation of success.
Would you stick with the Zoloft and augment with Seroquel (as was suggested to us by a specialist; only our local docs are skittish about this)? What's the scoop on pregabalin for SP as augmentation? Pindolol? Buspirone?
She's very sensitive to anything that raises NE too much - Strattera induced depression and psychostims increase anxiety.
I can't find too much on augmentation for SP.
I also would like to know if anyone knows of any programs for intensive CBT for SP for teens anywhere.
