Your 10 year history of searching for help and not finding relief is aggravating in itself. If your Doc cannot help you and you don't find yourself in a better place after two years under his/her care, CHANGE DOCTOR.
You Mom knows something needs to change, 'tough love' is adding to your stress. Can you get access to counseling or some routine which gives you positive feedback?
I don't know anything about your meds and your history, can't say what would benefit your situation.
