questions for the medicinally savvy

Hello,
I have GAD, OCD, Bipolar II, and Panic disorder. I take 30 MGS of Paxil, 900 MGS of Trileptal, 25 MGS of Trazodone (for sleep), and Klonopin and Zyprexa (2.5 MGS) for crisis situations.

I am not quite sure on what to do. The Paxil does not seem to be kicking my OCD,depression, and anxiety's butt. Don't get me wrong, it helps, but I am having alot of breakthrough symptoms of irrational thinking, intense overstimulation, and anxiety. It seems to intensify to crisis levels about once a month. When this happens, I take a zyprexa and it helps. I am hoping to get into my PDOC's office soon for some med changes.
I have tried numerous SSRIs and I definetely do get some level of relief from them, but certainly not enough. I don't have hypomanias anymore, Trileptal has wiped those out. Now I am left with bouts of agitated depression and OCD. Where should my next move be? Do I swap things around, or simply add things on? I really like the AP's, but after continuous use, I develop Akathisia, not the foot tapping kind, but the strong internal restless kind where no amount of walking will help. Also, the Zyprexa is notorious for weight gain.
Does anyone have any ideas on what I could do to fro logn term treatmet to whip these problems down to manageable levels? I am open to ideas and whatnot.
