I was on 60 mg per day of it, divided into three doses. It was great initially for MDD and SP, but then stopped working - I guess it caused all the dopamine to be released (faster than it could be produced), leading to a state of dopamine depletion and severe anergic depression.
I'd be interested to know if anyone else has experienced this, and found dexmethylphenidate or methylphenidate to be more sustainable in terms of therapeutic benefits? I was useless/dead for about two months after stopping the dextroamphetamine, but (dex)methylphenidate, at low CSF concentrations, only blocks dopamine reuptake, as opposed to causing dopamine release AND blocking reuptake. This would suggest, in theory, that it would provide more sustainable benefits - sort of like the differerence between the SSRIs and MDMA (or dexfenfluramine).
I'm in Australia (where dexmethylphenidate hasn't been marketed yet) but my doctor could arrange for it to be imported. I understand that the racemic methylphenidate causes more peripheral side effects and possesses less central potency (a la dextroamphetamine versus amphetamine).
From what I've heard from posters here, the manufacturer needs to develop a Concerta-style extended-release formulation of dexmethylphenidate, as each dose provides effects for only about two hours. As an aside, I also take sertraline 200 mg per day and clonazepam 4 mg per day; however, I found clomipramine to be the best antidepressant - sadly, the side effects led me to change to sertraline.
You people are a wealth of knowledge and experience, and I would really appreciate any insights that you could provide.
