Could be one of these three things or some combination thereof:1) Doxepin does have some ability to blockade serotonin 2A receptors, which will tend to counteract orgasm difficulties induced by serotonergic drugs like Effexor. However, this is a relatively weak effect compared to its powerful histamine blockade.
2) Effexor begins having more effect on blocking norepinephrine reuptake at dosages of 225 mg/day and above, and norepinephrine transmission is very important in the orgasm process.
3) Effexor also weakly blockades dopamine reuptake; it's theoretically possible this may have a positive effect on sexual function, again, more at higher dosages.
If I had to guess, I would say the 2nd possibility is the most likely one.
