Amphetamine and HYPOtension

I've been struggling with severe treatment resistant (BPII?) depression for 3 years. I've tried multiple meds in all classes, with what seems like all possible combinations. I lost count at 25 trials...
My problem is rarely nonresponse or partial response to ADs but rather very negative dysphoric response. The only positive responses I"ve had have been to stimulants and MAOIs. But each time I try them I develop severe fatigue and hypotension that is not strictly orthostatic. My mind is happy/focused/functional but after a week of treatment on a very low dose (10-15 mg Adderall) I can barely sit upright.
Has anyone had this seemingly paradoxical effect? Any clues/leads would be much appreciated.
