Have you tried augmenting the Parnate when it starts petering out? There are several meds that work well with Parnate. Also, how high of dose have you taken? From my research and talking with my doctor you can go up to 160mg as long as your blood pressure doesn't drop too much and you're not too dizzy. If you can handle the side effects though you may want to bump it higher. I was up to 100mg before I was too dizzy. I split the dose into three, four, and three pills every three hours. Also, Lithium is a common augmenting drug, or Lamictal (which I'm currently tapering up on). There are many others as well.

I have had treatment resistant depression for years as well. Since Parnate is the first thing in years to work I would be very hesitant to go off for a year.
