You need five weeks off (possibly more) of Prozac before beginning an MAOI. Two weeks is usually plenty for the others, and one week off of Effexor can be enough (based on psychopharm tips here). Have you considered selegeline? It is pushed rather hard off-label for ADHD (my pdoc has brochures in the waiting room talking about treating ADHD with selegiline from the manufacturer). Lithium isn't specifically contraindicated in the selegiline monograph-only meperidine.
