If you're not getting any results at 40 mg of Paxil, I would seriously question whether this is the right medication for you, before you raise the dose to a megadose like that. 40 is already a rather high dose. Also, since you had such negative results on Zoloft, I'm surprised your doctor would even be so focused on Paxil since they're so closely related... For some unknown reason, Paxil quit working for me after I was on it for 4 years straight. My doctor upped my dose from 20 to 40 to 60 to 80, and while I didn't experience any adverse effects other than the typical loss of sex drive, it did nothing to help at the higher doses either. You may want to ask your doc about Remeron. I've had amazingly good results taking that in conjunction with a smaller dose of Paxil. Good luck to you and don't give up. It took them 7 yrs to get the right meds combo for me but now I'm finally alright.
