Personally I don't see any problem in requiring whatever information you might find interesting or helpful. That's because, after all, each person here is completely anonymous unless they choose to not be.
I would think at your next presentation concerning the subject of the internet in psychiatry, having statistics on what countries and states people are communicating from would be an eye opener. Off the top of my head, I know we've had people from Brazil and Switzerland, among others. States ranging from coast to coast. And various provinces of Canada. I think that's pretty cool.
I would have no problem with you requiring more basic demographic knowledge; perhaps the psychiatric illness the patient deals with; whether they see a gp or a specialist, on and on; and then also have a section where the person can contribute whatever other information they want to voluntarily. As long as we don't give out our full real name, SSnumber, complete address, etc, we are completely and safely anonymous.
