Katherine, if it is hard for you to commit to not trying to end your life, it sounds like you need to be in a safe place, where someone else can ensure your safety, until you can ensure your own safety.
Right now, the OCD, trichotillomania, etc. are not the most important thing. The most important thing is to address the despair so that you can commit to living.
I know that when I was suicidal, all my thinking was distorted. What ended up helping was handing over some of the thinking to people who were thinking more clearly than me, until I was stabilized.
